South Shropshire Community Mental Health Service is currently mid-way through a two year pilot in looking at creative and innovative ways to engage service users living with psychosis that are often hard to reach through traditional models of care.  We are looking to widen our MDT for this client group and would welcome applications from Nurses, Occupational Therapists or Social Workers who have a specific interest in offering a team-based approach to Assertive Engagement with this client group. 


The post will be fixed term until May 2027 


In the role you will be responsible for working in an identified adult community mental health team, delivering interventions across the South of Shropshire, dependent on skill and service need.  You will be responsible for key working of an identified caseload utilising a team approach to case management drawing on the strengths of all members of the team.

You will be responsible for undertaking clinical assessments, in order to establish the person’s needs, goals, risks and strengths in order to generate a collaborative care plan agreed with all involved. You will be responsible for delivering recovery focused interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties.

Main duties of the job

Take responsibility for the management of a hard to reach complex caseload, which includes, specialist assessment (within service specification), care planning in collaboration with service user.
•  Lead on daily huddles to ensure effective communication within the team and with wider teams and ensure all daily tasks are allocated within the team.
•  Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
•  Assess patient's individual care needs, develop, implement and evaluate programmes of care utilising patient related outcome measures (PROMs including Requol10 and Dialog)
•  Utilise flexibility and creativity to deliver a range of interventions in accordance with the agreed plan of care including; pharmacological and psychologically informed interventions.
•  Complete and contribute to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence. 
•  Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process
•   Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
