Job overview

An opportunity has arisen for a dedicated Advanced Physiotherapist or Occupational Therapist to work in the Frailty Assessment Unit (FAU) within County Hospital, with close integration into Emergency Medicine and front-door services. This role is designed to provide senior clinical leadership to patients presenting with frailty syndromes, supporting safe, person-centred and efficient care pathways from the Emergency Department (ED) into either early supported discharge or further inpatient care.

 FAU is a bespoke 6 bedded frailty assessment unit. You will work in an MDT setting. The MDT comprises of consultants, ACPs, nursing staff, dedicated therapists and IDH support.

 The post-holder will work closely with ED, Acute Medicine, Geriatrics and community services to support admission avoidance, timely CGA and holistic patient management.

Frailty services at County Hospital are currently rated 3rd in the country (out of 176), SAMIT 75+ data.

The role spans the Emergency Department, Frailty Assessment Unit and relevant SDEC / Acute Medicine areas. There will be opportunities for flexible working, training and development.

 The post holder will be encouraged to develop innovative ways of working and consolidate their leadership and management skills as well as overseeing the implementation of clinical audit recommendations and facilitation of service development. There are opportunities to contribute to a multidisciplinary approach frailty service within the trust.

Main duties of the job

Supervision will be provided by Principal Therapists and continuous development is supported through appraisal, supervision, internal training and external learning. 

 This post will be required to work 8-8 shifts. Therapy team also work within a 7 day shift system therefore the post holder will be required to work weekends and bank holidays.

Main duties of the job

·         To work as a specialised practitioner managing the clinical delivery of care in a specialised area.

·         To lead the clinical team within your area and organise staff and resources to meet service standards and demands.

·         Patients include the  acute and chronic respiratory, multi pathology / complex patients, neurological, cognitively and perceptually impaired

·         To be responsible for the provision of specialist training to therapy staff and other members of the MDT.

·         Use analytical and problem solving skills to interpret, analyse and evaluate complex information from a variety of sources

·         To provide cover for other teams within the Directorate based on your knowledge, skills and competencies in order to meet changing priorities and service demands.

Education and Qualifications

  • Degree in Physiotherapy or Occupational Therapy
  • Registered with HCPC
  • Evidence of personal and Professional development
Desirable criteria
  • Working towards a higher qualification relevant to speciality e.g Masters degree

Experience

Essential criteria
  • Trained clinical educator
  • Experience of teaching students, staff & other professional groups
  • Experience of working as a senior physiotherapist/Occupational Therapist within the speciality area
Desirable criteria
  • Experience gained within an acute setting
  • Involvement in audit/research
