We would like to invite you to apply for an exciting opportunity that has arisen in our division of Urgent & Emergency Care. We are seeking enthusiastic and motivated candidates to apply for a Trust Grade SHO post (equivalent to FY2-3, salary experience dependent). This role is not recognised as a formal training post, however, the training opportunities will be similar to those at the same level training posts.
The post holder will gain a wide range of experience in all aspects of work within the Acute Medicine Department. During this time, the post holder will work primarily in the Same Day Emergency Care (SDEC) unit and the Clinical Assessment Unit (CAU).
Applicants should ideally have at least 6 months experience working in the UK.

Main duties of the job
The post holder will be part of the medical team, supporting acute medical units. The service is multi-disciplinary, and the post holder is expected to work as part of a health professional team under the supervision of a Consultant Physician.
You will be appointed an Educational/Clinical Supervisor and are able to fill in a portfolio. There is an in-house training program for resident doctors as well as grand rounds and specialty training.
The post holder will:
- Be mainly based in SDEC (Same Day Emergency Care) and occasionally on CAU (Clinical Assessment Unit)
- When on CAU, attend morning board rounds and daily ward rounds
- Take part in SDEC weekend cover (1:3)
- Professional development through attendance at Acute Medicine and local speciality teaching, core medical training and grand rounds
- Undertake audit and quality improvement projects
- Developing interests in Acute Medicine specialty
- Work closely within the multidisciplinary teams to provide a high quality daily and general medicine services to patients in the local area and to support the Trust in achieving all national targets.

Detailed job description and main responsibilities
In order to ensure the smooth running of the service, the post holder will:
- Provide both in-patient and out-patient care for patients in accordance with the Trust’s contractual obligations.
- Participate in the supervision and training of resident medical staff (FY1s on CAU) and offer guidance and advice to the wider MDT
- Maintain accurate record keeping for every patient seen and communication made with General Practitioners and other agencies.
- Keep up to date in advances in current knowledge and practice
- Work with and implement service policies, guidelines and protocols
- Participate in Trust training programmes to improve the quality-of-service provision.
- Work within and demonstrate an understanding of the Clinical Governance agenda.
- Conduct or participate in regular audit projects
- Undertake other duties as may be considered reasonable and appropriate.
- Attend and participate in staff and group meetings as required
It is the duty of all doctors to practice and conduct themselves within the standards framework provided by the GMC.
The post holder will provide support to less experienced medical staff and nursing staff and be approachable when sought to provide advice on the clinic floor.
The post holder is expected to liaise with his/her colleagues in providing internal cover for Annual leave and sick leave and this may require some flexibility in the post holder’s hours.
The post holder will be allocated a CS/ES from amongst the Consultant body and there is a robust system of appraisal.
You must have experience in General (Internal) Medicine and be comfortable in the initial assessment and on-going treatment of acutely ill adult medical patients with a wide variety of pathologies. You will work under Consultant and middle grade supervision and within a dedicated team at all times. As a hospital and a major secondary centre, we offer a large variety of sub-specialty services providing an excellent learning environment.

Experience
Essential criteria
- Evidence of experience in a range of acute medical specialties, with experience of managing patients on unselected medical take during core training or equivalent.
- Completion of FY1 training of equivalent and minimum 3 months experience in Medicine at SHO level
Desirable criteria
- 6 months in Medicine at SHO level
Clinical Skills, knowledge & expertise
Essential criteria
- Demonstrates awareness of the basics of managing acute medical conditions, including emergencies, inpatients and outpatients • Appropriate knowledge base, and ability to apply sound clinical judgement to problems • Proficiency in a range of medical procedures (as defined by the core curriculum) as an indication of manual dexterity and hand-eye coordination • Able to work without direct supervision where appropriate • Able to prioritise clinical need • Able to maximise safety and minimise risk
Desirable criteria
- Evidence of some competences in the specialty as defined by the relevant curricula • Evidence of skills in the management of acute medical emergencies (e.g. ALERT, IMPACT certification) • Evidence of skills in the management of patients not requiring hospital admission • Evidence of procedural skills relevant to medical patients (clinical independence in central venous cannulation, chest drain insertion, joint aspiration, DC cardioversion, abdominal paracentesis)
