Job overview

We have an exciting opportunity for a suitably experienced learning disability nurse to come and work in our team as community practitioner. The post holder will be flexible, approachable and work within the wider health and social care system in South Tyneside supporting children with learning disabilities and their families. 

Main duties of the job

The successful candidate will carry a caseload of patients, offering assessment and interventions such as PBS and other behavioural approaches to the wider systems around learning disabled children, young people and their carers.

The band 6 community practitioner will also support the wider team with duty, supervision and mentoring of other colleagues and students and contribute to social care team meetings as well as being linked in with special education and SEND colleagues across the borough. 

Part of the role will also include supporting with transition into adult teams where appropriate. 

This is a varied and diverse, flexible role within a successful and dynamic team currently undergoing lots of exciting development and service improvement work. Detailed job description and main responsibilities

Please note to apply for this role you must have the right to work in the UK. CNTW NHS Foundation Trust will not provide sponsorship for this role.

Will carry a caseload of young people with complex needs relating to their learning disability.  Delivering appropriate, evidence based interventions and coordinating their care & treatment. 

Take part in the duty rota with other clinicians supporting with calls & queries 

Undertake PBS training (if not already qualified) 

Carry out clinical assessments following referrals from our partner agency Lifecycle  

Supervision and mentoring of junior colleagues and students 

Liaison with other services in the system.

Person specification

Qualifications

Essential criteria
  • Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
  • Mentor Training.
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
Desirable criteria
  • Post registration qualification in psychological or relevant intervention
  • Mentorship Qualification

Experience

Essential criteria
  • Clinical experience relevant to post
  • 12 -24 months post qualifying experience
  • Effective interpersonal skills including de-escalation
  • Experience of risk assessment and management
Desirable criteria
  • Experience of working within or alongside educational settings

Skills

Essential criteria
  • Demonstration of ability to carry out assessment of complex mental health needs
  • Evidence of ability to maintain accurate patient records
  • Ability to work autonomously as well as within a team
  • Demonstrable experience and skill in safeguarding/risk assessment and ensuring appropriate escalation
Desirable criteria
  • PBS qualification or willing to work towards.
