Job overview

An exciting opportunity to join the Community mental health - Older adult service, an expanding team working with patients with both dementia and functional disorders. 

The role of the Memory assessment clinician is open to CPNs and Occupational Therapists interested in developing their skills in Memory assessment work. The role will involve working predominantly with patient presenting with memory difficulties, completing assessments of cognition and supporting the diagnostic process. However, as the team grows and develops may include some work with patients presenting with functional illness also. 

Main duties of the job

The focus of this role will be on completing the following main duties;

  • Memory Assessments within clinic or patients home environment, dependent on patient need. 
  • Timely diagnosis, following close joint working with Consultant psychiatrists. 
  • Pharmacological Treatment, as per guidance from Consultant psychiatrist. As appropriate depending on profession. 
  • Post diagnosis support and sign posting. 
  • Monitoring and reviewing of patients started on treatment.
  • Contribution to duty system. 
  • Assessment of risks at home and support to address/ mitigate utilising MDT approach.

Qualifications

Essential criteria
  • Relevant Professional Qualification ie. RMN, DipCOT
  • Current unrestricted registration with HCPC/NMC

Experience

Essential criteria
  • Substantial experience in a Community Mental health care setting
  • Developed specialist knowledge gained within a similar environment over a considerable period of time
  • Significant experience of undertaking assessment, formulation, risk assessment and positive risk management
  • Proven experience of people management

Other

Essential criteria
  • Must have full driving licence and access to a motor vehicle for business use
