Coventry and Rugby Community Learning Disability Team have an exciting opportunity to join the team to role will primarily be working in  health facilitation, supporting  people with a learning disability both to avoid acute hospital admissions and to support with complex discharges. jointly working with the acute Hospital L/D liaison Nurse, local GP's community services, alongside Community learning disability Nursing roles

Main duties of the job

To promote the needs and rights of people with learning disabilities to other agencies and the general public.

To identify and evaluate specific client needs through specialist assessment.

To assist in the development of new services and projects within the community to meet identified client’s needs, including those with complex needs and/or behaviours that challenge.

To participate as an active member of the Learning Disability Nursing Team and establish and maintain effective communication with other agencies and professionals.

To act as a health facilitator for people with a learning disability both to avoid acute hospital admissions and to support with complex discharges, jointly working with the acute Hospital L/D liaison Nurse

  • Diploma / Degree holder
  • Teaching/mentoring qualification
  • Evidence of ongoing CPD

Experience

Essential criteria
  • RNLD with a substantial post registration experience at Band 5
  • Experience of working in multi agency teams with multi-disciplinary colleagues
  • Experience of working as a community nurse (PWLD)

Other

Essential criteria
  • Independent means of transport
  • Full UK DL
