Job overview
We currently have an opportunity for a band 6 clinician to work with facilitated supported discharge (FSD) within Brighton & Hove Urgent Care.
The local authority have granted an extension of winter monies funding which has enabled continuation of the enhanced FSD provision over the next 6 months
Therefore we are looking for an additional band 6 clinician to work alongside our current FSD practitioner which will enable enhanced cover across urgent care and inpatient pathway within Brighton and Hove.
Main duties of the job
The role will involve working as part of an MDT and this will include in reach with inpatient wards, Haven, MHLT and ECR providers. The role will also involve developing and maintaining relationships with our pathways practitioners and MDIST colleagues who provide in reach for SPFT patients admitted to non SPFT hospitals.
Part of the role will also involve attending MDT's, ward reviews and supporting staff in identifying potential and appropriate referrals to Crisis Team.
We are looking for an enthusiastic and dedicated practitioner who is able to work autonomously as well as an integrated part of the MDT. Working patterns and hours can be negotiated. We are looking for consistent cover over a 6 month period.
Ideally, you will have experience of working within urgent care and have an awareness of services and teams within the Brighton and Hove Division.

REQUIREMENTS
In order to be a successful Urgent Care Practitioner in our team, you must:
- Be a Registered Nurse with current registration (Adult/Mental Health/LD or equivalent) appropriate to the job role OR Social Worker OR Occupational Therapist with relevant professional registration
- Significant post qualifying experience in the relevant care group
- Experience of working alongside people with complex health problems in the designated care group
- Experience of conducting clinical assessments including risk assessments
It would be advantageous is you have:
- Previous experience working in acute services, home treatment teams or Liaison Psychiatry.
