The post holder will work as part of the diabetes integrated multi-disciplinary team providing clinical support to community based intermediate diabetes care clinics, specialist diabetes clinics, and the in-patient population with diabetes.  The post-holder will be responsible for the management of a defined caseload of patients with Diabetes, providing specialist nursing support, advice, education and/or care to that specific group. They will also participate in the delivery of structured patient diabetes education sessions and healthcare professional education both for community and hospital-based staff.  Clinical care will be delivered at numerous primary and secondary care locations across Bradford.

The post holder will work as part of innovative diabetes nursing team, supporting the implementation of Think Glucose, a national initiative designed to improve the care of in-patients with diabetes, across Bradford Teaching Hospitals Foundation Trust (BTHFT) They will carry continuing responsibility for the assessment of needs, the development, implementation and evaluation of programmes of care and the setting of standards within BTHFT. 

Main duties of the job

  • Facilitate a smooth journey from admission to discharge for the patient with Diabetes admitted to a ward within Bradford Teaching Hospitals.
  • The post-holder will have responsibility for a defined caseload of and will provide an effective and high quality patient-focused service that is sensitive to the needs of users and providers. 
  • Provide clinical, social and psychological support to their patients/service-users using excellent communication skills, education and health promotion to enable them to reach their optimum level of independence and health.
  • Facilitate in the development of the existing diabetes service, in line with local and National Guidance.
  • Develop close working relationships with all members of the Diabetes integrated multidisciplinary team, based in both secondary and primary care.

The diabetes specialist team delivers a range of patient orientated structured education sessions of which many have been accredited by a national scheme (Qismet) and the post holder will take an active part in this service. 

The post holder will take daily responsibility for managing a defined caseload, with the support of existing nursing and medical staff. They will utilise skills to assess and identify patient needs and implement delivery of appropriate individualised care and treatment for people with diabetes.

  • Significant post-registration experience, including experience of working in a relevant clinical specialty

Qualifications

  • Evidence of current CPD
