Job overview
The post holder will be an integral member of the Diabetes & Endocrinology Services and is responsible for the operational and clinical management of the Diabetes & Endocrinology Services, including accountability for the patients' experience of care and the clinical environment. To provide strong clinical and operational leadership to staff and patient care, and work alongside the medical director and senior management team. The post holder will also and have regular contact with West Yorkshire ICB and other local NHS trusts.
The post holder will also work as an autonomous advanced practitioner providing a high standard of assessment, diagnosis, treatment and highly specialist expert advice to patients and carers.
The post will be available as a development role - Deputy Diabetes Lead Nurse Advanced Practitioner (Band 8a) for a suitable candidate who has some clinical and operational management experience, but insufficient for the Lead Nurse position.
Main duties of the job
The post holder is responsible for the strategic and professional direction of the Diabetes service across Bradford Teaching Hospitals NHS Foundation Trust.
The post holder will provide expert clinical advice and leadership to the integrated clinical team, drawing on clinical knowledge to undertake advanced assessment, diagnosis and treatment. They will manage clinical caseloads of patients in and across primary care.
The post holder will work closely with consultant teams to deliver high standards clinical management of patients.
The role will provide high level expert advice to Primary Care Practitioners to determine the most appropriate plan of care for patients referred into the service.
The post holder will provide support teaching and education programme to medical, nursing staff and patients.
The post holder will be responsible for the development of integrated care pathways and link with social care and primary care colleagues to support admission avoidance in liaison with other clinicians and partner organisations.
The post holder will lead the delivery of high quality, efficient services, and will be responsible for the operational and financial management, performance monitoring, professional leadership, service improvement, and development of the diabetes service.
The post holder will be the lead in delivering diabetes care that is evidence based and continually ensuring practice is reviewed and evaluated to ensure the Trust delivers safe quality patient care.

- 1st Level Registered Nurse

- Hold or working towards a master’s degree in a nursing or leadership; related subject or equivalent experience
- Evidence of recent professional development
