Job overview
• As a Clinical Nurse Specialist, contribute to further development of the Haematology
Service throughout the integrated Calderdale and Huddersfield NHS Trust
• Working together with other members of the multidisciplinary team, provide a specialist
nursing service encompassing specialist care, advice, support and information to
haematology patients, their carers and other health professionals within the trust
• Empower and enable other professionals to meet the needs of haematology patients and
their families.
• Act as a resource for members of the multi-disciplinary team in relation to the management
of haematology patients
• As a Clinical Nurse Specialist, contribute to further development of the Haematology
Service throughout the integrated Calderdale and Huddersfield NHS Trust
• To work within the NMC code of professional practice, and within policies, procedures and
guidelines of Calderdale & Huddersfield NHS Trust.
• Working together with other members of the multidisciplinary team, provide a specialist
nursing service encompassing specialist care, advice, support and information to
haematology patients, their carers and other health professional within the trust
Main duties of the job
Clinical duties
• Provide specialist advice, information and support for the users of the service, through diagnosis, treatment and follow –up.
• Establish and maintain effective working relationships with other healthcare professionals across the Trust involved in the management of haematology patients
• Act as a keyworker supporting patients through the haematology pathway including diagnosis, care planning, follow-up and discharge / end of life care planning.
• Identify patients physical, social and psychological needs and refer to appropriate agencies as required
• Provide advice, information and support to all haematology patients and their families in the community by providing telephone advice when needed In conjunction with the multidisciplinary team, build upon established links with primary care to develop comprehensive, integrated care pathways for haematology patients
• Liaise with tertiary cancer centre in co-ordinating patient care, e.g. arranging stem cell harvest, pre autograft / allograft investigations
• To be/become a non-medical prescriber.
• To be able/become an authoriser of blood products

- Registered General Nurse
- Mentorship / Preceptorship Qualification
KNOWLEDGE, EXPERIENCE & EXPERTISE

- Substantial post registration experience
- Experience of working in the specialty of oncology and/or haematology
- Evidence of continuing professional development
- Evidence of recent education in relevant field
- Evidence of maintaining high level skills in assessment and management of haematological patient care.
- Knowledge of nursing conditions and treatments

- Venepuncture Cannulation skills
