About the Role

In this key role, you’ll take the lead in shaping and overseeing the delivery of high-quality harm reduction services. You'll guide a dedicated team, ensuring compassionate support reaches those who need it most—within our service and out in the community. You’ll  ensure best practice is maintained and continuously developed.

What You’ll Do

  • Lead and support the Harm Reduction Social Support Team, providing day-to-day guidance and supervision.

  • Ensure services are delivered with empathy, dignity, and in line with harm reduction best practice.

  • Deliver direct support to individuals who use substances, meeting them where they are, and helping them achieve improved health and social outcomes.

  • Build and maintain strong partnerships with local agencies, health services, and community organisations.

Main duties of the job

As a Senior Harm Reduction Social Support Worker, you will lead a team providing assertive outreach and enhanced social support to drug and alcohol users across Leeds. This includes delivering harm reduction interventions, addressing housing and benefits needs, and working flexibly across three Forward Leeds hubs and the wider community.

Key responsibilities include:

  • Providing supervision, appraisals, and day-to-day support to the Harm Reduction Social Support Team.

  • Coordinating staff rotas and ensuring adequate staffing levels.

  • Supporting team meetings and fostering reflective practice.

  • Monitoring team performance against targets, producing monthly reports, and identifying service gaps.

  • Delivering needle and syringe provision and specialist harm reduction advice, including safer injecting practices and overdose prevention (e.g., Naloxone training).

  • Supporting clients using all types of substances, including those not accessing structured treatment.

  • Managing a caseload, developing person-centred support plans, and working closely with partner agencies to meet health and social needs.

  • Providing harm reduction training to professionals and contributing to multi-agency outreach work, including evenings and mornings.

Detailed job description and main responsibilities

As a Senior Harm Reduction Social Support Worker, you will lead a responsive team supporting people who use drugs and alcohol across Leeds, delivering high-impact harm reduction interventions and holistic social support. You’ll provide operational leadership, direct client support, and play a critical role in service development, ensuring quality, safety, and collaboration across the Forward Leeds partnership.

Leadership & Team Support

  • Supervise, support, and appraise Harm Reduction Social Support Workers.

  • Manage team rotas and ensure sufficient staffing across services.

  • Facilitate regular team meetings and promote reflective practice.

  • Support the Service and Team Managers in meeting partnership targets and delivering high-quality service.

  • Monitor team activity, produce performance reports, and identify service gaps.

Harm Reduction Delivery

  • Deliver needle and syringe provision and ensure tailored harm reduction advice is provided (e.g., safer injecting, overdose prevention).

  • Provide Naloxone training and overdose information to service users.

  • Support people using a range of substances, including those not in treatment.

  • Collaborate with partner agencies for outreach harm reduction activities, including evenings and mornings.

  • Train professionals in harm reduction principles and safer injecting.

Client Support & Case Management

  • Hold a caseload and develop person-centred support plans covering housing, benefits, and wider social needs.

  • Deliver drop-in support sessions, brief interventions, signposting, and onward referrals.

  • Prioritise Assertive Outreach tasks (e.g., welfare checks) and manage a dynamic workload based on risk and need.

  • Ensure client voices are central to care planning, advocating for their rights and supporting co-production wherever possible.

Partnership & Communication

  • Work collaboratively across all Forward Leeds hubs and with external partners.

  • Maintain clear, timely, and accurate records and reports in line with data protection and confidentiality policies.

  • Promote strong communication within the team, across the organisation, and with external stakeholders.

Service Quality, Safeguarding & Development

  • Adhere to safeguarding procedures for children and vulnerable adults, acting swiftly on concerns.

  • Contribute to service development by identifying new opportunities and improving care delivery.

  • Participate in supervision, peer support, and ongoing professional development.
