At Forward Leeds, we’re looking for a Harm Reduction Social Support Worker to join our dedicated Harm Reduction Team. You’ll be at the heart of our mission—supporting individuals who use drugs and alcohol to live healthier, safer, and more empowered lives.

Provide non-judgmental, person-centered support to people using substances.

Main duties of the job
In this dynamic role, you’ll deliver hands-on harm reduction support while advocating for some of the most vulnerable members of our community—balancing frontline care with partnership working and case management.
Key Responsibilities:

Provide needle and syringe provision, safer injecting advice, and harm reduction equipment tailored to individual needs.

Deliver overdose prevention advice and Naloxone training to service users.
-
Offer specialist guidance on safer drug use and alcohol-related harm.
-
Support safe return of injecting equipment by building trust with service users.

Join partner agencies for outreach sessions, including early mornings and evenings.
-
Manage a caseload, developing tailored support plans based on health and social needs.
-
Provide practical advice on housing, benefits, and welfare.

Detailed job description and main responsibilities

Train professionals in harm reduction and safer injecting practices.

Support safe return of injecting equipment by building trust with service users.

Join partner agencies for outreach sessions, including early mornings and evenings.

Manage a caseload, developing tailored support plans based on health and social needs.

Provide practical advice on housing, benefits, and welfare.

Run drop-in sessions—offering brief interventions, advice, and referrals.
-
Respond promptly to welfare checks and Assertive Outreach tasks.

Prioritise workloads to address urgent needs effectively.
-
Work within referral pathways and with partner organisations.
-
Offer flexible support—including occasional evenings, weekends, and bank holidays.
-
Respect client rights and advocate for their voice in care planning.
-
Encourage service user participation and co-production in service design.
-
Keep accurate records and provide reports as needed.

Recognise and act on safeguarding concerns in line with policy.
