Community Urgent Response and NHS@Home Pharmacist

Band 8a

Great Western Hospitals NHS Foundation Trust is a unique organisation, serving as the sole integrated provider in the Bath and North East Somerset, Swindon, and Wiltshire system. Our scope extends beyond boundaries as we oversee adult community services in Swindon and provide acute care from the Great Western Hospital.

Our 5,500 staff have 1.2m patient contacts a year, with 13,000 operations carried out in our Theatres, around 4,000 babies born, and our emergency attendances are now well over 100,000 per year. Those numbers only tell part of our story about what an exciting time it is to join Great Western Hospitals.

We are cultivating a culture that fosters our ambitions, prioritising equality, diversion, inclusion, wellbeing, and leadership. We aim to create an environment where every member of our team can truly thrive.

The Trust is a Disability Confident Employer; all applicants who have a disability can opt to be considered under the guaranteed interview scheme, which means that, providing they meet the minimum essential criteria for the post, they will be offered an interview. We are also proudly accredited as a Veteran Aware organisation and have signed The Armed Forces Covenant.

If we receive a high volume of applications, we may close adverts prior to the published closing date. Therefore, we advise you to complete and submit your application as soon as possible.

Job overview

We are looking for an enthusiastic Pharmacist who will embrace working within the community setting within Swindon. We are looking for a Senior Pharmacist (band 8a) but would also consider a development position (band 7-8a) if this is something you are looking towards.

You will be a Senior Pharmacist supporting the exciting Urgent Response and NHS@Home service in Swindon which incorporates Virtual Wards. A virtual ward is a safe and efficient alternative to NHS bedded care providing treatment and care in a patient’s own home. This supports early discharge out of hospital and prevents avoidable admissions. You will provide a highly specialist clinical pharmacy service to virtual ward patients, optimising medicines and ensuring agreed changes in medication are implemented and sustained. This could involve remote consultations, home visits and attending multidisciplinary team meetings. You will be involved in developing and delivering safe and effective pathways of care to patients for a range of conditions. You will also be a key part of a collaborative approach to virtual wards across the BaNES, Swindon and Wiltshire (BSW) Integrated Care Board (ICB).

As a Senior Pharmacist you may be required to support the wider pharmacy team within Great Western Hospitals as the needs require. This may include clinical pharmacy ward and dispensary support.  Please see the job description for further details.

Main duties of the job

  • Carry out medicine reviews virtually and face to face in patient homes, as necessary
  • Work as an independent prescriber (when trained), optimising medicines and promoting appropriate evidence-based and cost-effective prescribing
  • Utilise clinical assessment skills, where competent and appropriate, to support clinical review of patients within the Virtual ward
  • Provide professional advice and support during consultant and multidisciplinary Virtual Ward rounds
  • Assist in the management of prescribing across the primary/secondary care interface to promote a unified and coordinated approach
  • Developing the pharmacy virtual ward service, in line with national standards
  • Support patients and their carers in respect of their medication, including education for patients/carers and medicines advice to health and social care providers
  • Support and develop the admission and discharge process to and from Urgent Response and NHS@Home services to ensure safe transfer of care in relation to medicines.
  • Co-ordinate the day-to-day activities of the pharmacy service, supervising and line managing pharmacy technicians and rotational pharmacists within the service.
