Job overview

An exciting opportunity has arisen for an experienced and enthusiastic pharmacist to join the innovative virtual ward team at University Hospitals Dorset. This is a great opportunity to work in a dynamic and evolving environment, providing pharmaceutical expertise to support the delivery of safe and effective patient care. We are looking for a clinical pharmacist to join our newly established virtual ward pharmacy team, working alongside the Lead Virtual Ward pharmacist to design, enhance and deliver high quality pharmacy services to patients on the virtual wards.

You will be responsible for providing a highly specialist clinical pharmacy service to patients. You will work closely with the multi-disciplinary team to support early discharges out of hospital and prevent avoidable admissions in a range of specialties which include child health, older people medicine, respiratory, cardiology and antimicrobial therapy. Additional specialties may be included as the virtual ward service expands.

You will be supported in your personal development, and you will be actively encouraged to evolve your clinical, professional and leadership skills. 

Main duties of the job

Main duties of the job 

  • Provide specialist clinical pharmacy services to the virtual wards, ensuring the safe and appropriate use of medicines.
  • Conduct comprehensive medication reviews and provide evidence-based recommendations to improve patient outcomes.
  • Collaborate with the multidisciplinary team to optimise medicines management, including medicines reconciliation, prescribing, and monitoring.
  • Support the development and implementation of clinical guidelines, protocols and policies related to pharmacy and medicines management for virtual wards.
  • Build relationships and work with internal stakeholders and external partners to support the delivery of high-quality pharmaceutical care to virtual ward patients. Collaborate with other healthcare professionals and contribute to the overall patient care strategy.
  • Play a role in the on-going development of pharmacy services on the virtual wards. Contribute to audits relating to medicines management and other aspects of service delivery to virtual wards and suggest improvements, and stay updated with best practices.
  • Act as a role model and pharmacy ambassador, and demonstrate leadership qualities. Mentor and line manage rotational pharmacists, helping them develop their skills and knowledge. Develop and deliver appropriate training and education as needed to the pharmacy team, other healthcare professionals, patients and carers on medicines-related topics.

This role will require one late finish per week, weekend shifts 1 in 5 (either Saturday or Sunday) and approximately one Bank Holiday shift per year. This role includes on-call duties, the roster is approximately 1 week in 9 weeks.

Base location: Royal Bournemouth & Poole Hospital. Please note that cross-site working is expected.

Qualifications

Essential criteria
  • Master’s degree in Pharmacy or equivalent
  • Certificate in Clinical Pharmacy or evidence of equivalent experience
  • Non-Medical Prescribing qualification or prepared to train for this
  • Pharmacist registration with General Pharmaceutical Council (GPhC)
Desirable criteria
  • Diploma in Clinical Pharmacy or equivalent

Experience

Essential criteria
  • 2 years NHS hospital experience
  • Clinical audit / Drug Use Review projects
Desirable criteria
  • Mentoring / training of staff

Other

Essential criteria
  • Good organisational abilities and able to prioritise work
  • Ability to provide complex advice and guidance requiring analysis, interpretation and comparison of clinical options
  • Good clinical knowledge of current therapeutics and pharmacy practice
