Job overview

An exciting opportunity has arisen for a dynamic, motivated individual to join the Frailty Team as a Clinical Frailty Assistant. The individual must be highly motivated with a friendly and approachable manner and possess excellent communication skills. The Clinical Frailty Assistant is expected to support healthcare professionals in delivering high-quality patient care. They assist with basic clinical tasks such as observations, mobility support, and hygiene needs. The role includes recording patient information and reporting changes in condition. They help maintain a clean, safe, and comfortable environment for patients. Working as part of a multidisciplinary team, they ensure effective communication and continuity of care. Empathy, reliability, and strong interpersonal skills are key to this role with special attention to understanding the needs of a frail older person. This position contributes to smooth daily operations of the frailty team and positive patient experiences.

Main duties of the job

The post holder will provide support to the Frailty team and ensure the flow of the frailty service.

 They will work alongside the Multidisciplinary Team (MDT) to ensure that the effective admission to the FAU (Frailty Assessment Unit) and discharge planning are maintained in order to provide a smooth, timely patient discharge from hospital.

Education and Training

Essential criteria
  • Evidence of CPD
  • NVQ Level 3 in health and social care or relevant experience
Desirable criteria

  • Excellent knowledge of Frailty and the needs of older vulnerable adults
  • Good communication skills both written and verbal
  • Basic IT skills and knowledge of applications.
