Main duties of the job

The focus of this Higher Specialty Trainee level fellowship post is to lead on project that aims to further develop the hospital at home team.

Hospital at home is an initiative that has been developed at Darent Valley Hospital to provide short-term, acute care within patients’ homes (including residential and care homes) to manage acute problems and safely reduce the need for in-patient hospital care. It is led by a senior multidisciplinary team who have expertise in hospital based acute care, and aims to manage the holistic needs of the patients.

This fellowship will enable a senior trainee to work with the multidisciplinary team to further develop the project. This will embed service development and leadership skills, working with the wider hospital leadership team. This would include being a stakeholder in the hospital-wide programme to improve patient flow, attendance at patient safety reporting meetings and complaints responses.
