We are thrilled to offer this role, working with stroke and neurology services with our new Hyperacute Stroke Unit and Frailty services at Dartford & Gravesham NHS Trust.

Our innovative clinical pharmacy team are looking for a self-motivated clinical pharmacist to join us in our stroke and neurology services in our new Hyperacute Stroke Unit. You will work closely with our neurology team and lead clinical pharmacists to develop and deliver pharmaceutical services to our stroke and neurology patients in both an inpatient and outpatient setting. You will also lead and develop our clinical pharmacy frailty service with a particular focus on falls prevention and transfer of care on discharge.

Main duties of the job

Applicants must be currently registered with the GPhC as a pharmacist and have an interest in stroke/neurology and frailty medicine. Applicants should also be willing to work toward (if not already achieved) an independent prescribing qualification. Previous UK hospital pharmacy experience is desirable.

You will have regular clinical commitment working with our dedicated medicine management trained and accuracy checking pharmacy technicians who are fully integrated into the dispensary and ward service providing a full team approach to support near-patient medicines optimisation on the wards.

Detailed job description and main responsibilities

The postholder will have responsibility for leadership and development of clinical pharmacy services to stroke and neurology services including the Hyperacute Stroke Unit (HASU) and all Trust neurology services.

They will provide support to the Lead Clinical Pharmacist on the strategic development of clinical ward pharmacy standards to stroke and neurology services in line with the RPS Hospital Standards and also support the development and implementation of medicine management strategies, providing pharmacy input to aid in the stroke and neurology services’ key performance indicators.

They will lead and develop clinical pharmacy services to Frailty and Ageing and Health including providing expert opinion on medication related issues with regards to falls prevention and transfer of care on discharge.

EDUCATION AND TRAINING

Essential criteria
  • GPhC registration as a pharmacist
  • Post graduate Diploma in Clinical Pharmacy or equivalent.
  • Independent prescriber (or willing to work towards)
  • Evidence of CPD
Desirable criteria
  • Member of the Royal Pharmaceutical Society
  • Management and leadership qualification
  • MSc or equivalent postgraduate qualification in Clinical Pharmacy or other appropriate discipline
  • Practice/Educational Supervisor Accreditation or equivalent teaching accreditation

EXPERIENCE

Essential criteria
  • Experience working as a band 7 Pharmacist
  • Experience of writing protocols and standards
  • Experience of working in a multi-disciplinary clinical pharmacy setting
Desirable criteria
  • Hospital Pharmacy Experience
  • Clinical Pharmacy teaching skills.
  • Demonstrable use of audit/research to improve practice

KNOWLEDGE

Essential criteria
  • Demonstrates knowledge of national and local guidelines relating to specialism that need to be adhered to
  • Advanced computer literacy including Word, Excel and PowerPoint
  • Budgetary control skills
  • Awareness of current practice and policy affecting pharmacy practice in the NHS & understanding the NHS structure
  • Broad pharmaceutical knowledge relating to general medicine and core medical specialities
  • Good communication skills
  • Good interpersonal, including influencing skills.
Desirable criteria
  • JAC computer skills
  • Incident investigation/ root cause analysis
  • Risk management
  • Experience and knowledge of stroke and neurology medication use
