Care and Treatment Review Case Manager – Band 6

Transforming Care Team Secondment until 31st March 2020

If you are looking for new challenges, an opportunity has arisen for a dynamic and motivated individual to undertake the role of Care and Treatment Case Manager. The post holder will be responsible for undertaking, monitoring and reviewing care and treatment reviews for individuals with complex learning disabilities, autism or both across Lancashire and South Cumbria.   It will involve chairing, planning and monitoring of care and treatment reviews. The post holder will be the main point of contact for health and social care colleagues as well as providers, service users, family and carers. Evidence of extensive multi-disciplinary/multi-agency working is crucial to the role. The post will be based at Jubilee House in Leyland and will require extensive travelling across Lancashire, South Cumbria and beyond.

You must have appropriate UK professional registration.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
