An exciting opportunity has arisen for a highly motivated, innovative and committed Band 6 Complex Discharge Practitioner to join our established multidisciplinary team working within Multi-Agency Integrated Discharge Team. 

The successful candidate will need to have excellent organisational and negotiation skills, alongside a strong knowledge of discharge processes for patients with complex health and social care needs. An understanding of the National Framework for Continuing Healthcare and Funded Nursing Care and a working knowledge of safeguarding principles are essential. Excellent communication skills are also required to enable the post holder to work sensitively with patients and families during challenging times. The ability to coordinate the multiple services involved in supporting complex discharges is also key.

Person requirements

You should be a registered health or social care professional (e.g. NMC, HCPC or Social Work England registered) with relevant experience, including involvement in complex discharge planning for patients with complex needs.

 

 

Main duties of the job

This is a challenging and rewarding role requiring the post holder to work collaboratively with patients, families, and a wide range of clinical and non-clinical staff, including colleagues from the Local Authority, Continuing Healthcare/Personalised Commissioning, Bradford District Care NHS Foundation Trust, and various multi-agency support teams.

The post holder will work across Bradford Royal Infirmary, St Luke's Hospital, and community hospital sites.

The role is worked over 7 days per week, with participation in a bank holiday rota.

Detailed job description and main responsibilities

Responsible for the provision of specialist discharge planning support for the most complex group of patients with complex health needs on discharge from the acute and community hospitals.

 The role entails working across organisations and links with Social Services and Primary Care Trusts and is key in working with family and carers to support the discharge process.

  The post is an integral part of a multi-disciplinary team which contributes to the safe and effective discharge of patients with complex needs from all areas of the Trust.

The role also works at the interface with Bradford District Care Trust and Social Services, the Voluntary Sector and other external organisations.  It has a responsibility for assessing need for packages of care to be met from a variety of financial sources and requires in depth knowledge of criteria for many Health and Social Care Services.

Experience

Essential criteria
  • Working in a specialty dealing with complex discharges.
  • Practical experience of working in an acute NHS setting.
  • Experience of working in a MDT and across organisational and professional boundaries
Desirable criteria
  • Experience in policy development
  • Experience of developing innovative practice

Skills

Essential criteria
  • Verbal and written command of English
  • Computer literate – ability to use word/Excel/Power Point

Knowledge

Essential criteria
  • Understanding of discharge process
  • Understanding of Information Governance and Confidentiality
Desirable criteria
  • Knowledge of change management processes

Qualifications

Essential criteria
  • Registered health or social care professional (NMC, HCPC or Social Work England registered)
  • Health or social care related degree or equivalent experience
  • Teaching/Practice Assessor qualification or willingness to work towards
  • Evidence of current CPD
Desirable criteria
  • Post-qualification training in a relevant speciality
